Your bridge between Hospital & Home.
If you or a loved one are recovering from injury, surgery, or other acute illnesses, the Transitional Care Center at Episcopal Homes of Minnesota offers guests a place to regain strength at a comfortable pace with highly trained clinical staff. While recovery from surgery or illness can be quite challenging, our priority for short-term rehab transitional care is to ensure you are provided with a quality rehabilitation plan while remaining comfortable in a supportive and healing environment.
In between Physical, Occupational or Speech therapy sessions, guests recuperate in private rooms (with no private room surcharge), with an electric bed and electric recliner chair, made-to-order menu, and countless campus amenities. Our highly-specialized therapists, experienced nurses, and clinical team ensure you’re headed in the right direction: home!
