Episcopal Homes is launching NeighborCare, an innovative new home care model that is designed to make in-home support services more affordable and accessible for older adults. Unlike traditional models of home care, teams of care professionals are assigned to serve multiple clients each day within specific neighborhoods. Support is offered 24 hours a day by team members who remain within close proximity and are available, if needed, through a push of a button should an unplanned difficulty arise.
Unlike the one size fits all model common today requiring three- to four-hour care minimums, NeighborCare offers support in smaller increments. By helping clients with routine scheduled tasks and removing small barriers, elders can remain safe and independent in their homes. NeighborCare also offers intensive home care assistance. By providing a range of support options rather than just one, NeighborCare reduces costs and improves access to care.
“This service is critical as area residents have clearly said they desire to age in their neighborhoods among social support networks,” says Karen Vento, NeighborCare program manager. “This model helps them do that while saving funds and delaying possible nursing home placement. We are very excited to begin serving seniors in an innovative way.”
The new service is made possible by a Live Well at Home grant from the State of Minnesota Department of Human Services. Episcopal Homes is one of 45 organizations across Minnesota to receive a grant that is aimed at helping older adults maintain their health, independence and community involvement.
